About Loyalhanna Care Center

Loyalhanna Care Center is a 143-bed skilled nursing and rehabilitation facility located in Latrobe, Pennsylvania. We proudly serve the residents of Westmoreland County and surrounding communities, providing compassionate, person-centered skilled nursing, short-term rehab, and long-term care.

We are seeking a Director of Nursing (DON) to join our leadership team and guide our nursing department with professionalism, teamwork, and a strong focus on quality resident care.

Position Summary

The Director of Nursing is responsible for the overall management and leadership of the nursing department. This role ensures that resident care and nursing services meet the highest standards in compliance with CMS, Pennsylvania Department of Health, and facility policies.

As a key member of our leadership team, the DON promotes a culture of excellence, integrity, and accountability throughout the facility.

Responsibilities

  • Provide leadership and direction to all nursing staff (RNs, LPNs, CNAs).
  • Ensure compliance with all federal, state, and facility regulations.
  • Oversee daily clinical operations, staffing, and resident care delivery.
  • Develop and implement nursing policies and quality improvement initiatives.
  • Participate in QAPI (Quality Assurance and Performance Improvement) and infection control programs.
  • Monitor clinical documentation for accuracy and completeness.
  • Lead staff education, development, and performance evaluations.
  • Collaborate with physicians, therapists, and other departments to ensure continuity of care.
  • Maintain readiness for Department of Health inspections and ensure timely follow-up on any deficiencies.
  • Serve as a professional role model and mentor for the nursing team.

Qualifications

  • Active RN license in the State of Pennsylvania (required)
  • Minimum 3–5 years of nursing leadership experience in a skilled nursing or long-term care facility preferred but not required
  • Strong knowledge of CMS and Pennsylvania Department of Health regulations
  • Excellent leadership, communication, and problem-solving skills
  • Proven ability to manage staffing, budgeting, and clinical quality programs
  • Proficiency in electronic health records (EHR) systems
  • Commitment to resident-centered care and staff engagement

Preferred

  • BSN or MSN degree
  • Experience managing a 100 bed skilled nursing facility
  • Background in infection control, wound care, or compliance

Education

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university.

Experience

  • Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility preferred but not required

Specific Requirements

  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be able to read, write, speak, and understand the English language.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.

Physical and Sensory Requirements
(With or Without the Aid of Mechanical Devices)

  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
