Responsibilities
- Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
- Develop individualized treatment plans based on patient needs and goals.
- Provide skilled therapeutic interventions for communication, cognition, voice, and dysphagia disorders.
- Complete timely and accurate documentation in accordance with regulatory and payer requirements.
- Educate patients, families, caregivers, and facility staff on treatment strategies and recommendations.
- Collaborate with physicians, nursing staff, rehabilitation professionals, and other healthcare providers.
- Participate in care planning meetings and interdisciplinary team conferences.
- Ensure compliance with Medicare, Medicaid, state, and company policies and procedures.
- Travel between assigned facilities, patient homes, and outpatient locations as needed.
- Master’s Degree in Speech-Language Pathology from an accredited program.
- Current state Speech-Language Pathologist license.
- Certificate of Clinical Competence (CCC-SLP) preferred
- Current CPR certification or ability to obtain upon hire.
- Valid driver’s license, reliable transportation, and automobile insurance.
- Strong clinical, communication, and organizational skills.
- Experience in geriatrics, skilled nursing, home health, or outpatient rehabilitation preferred.
