Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants in accordance with federal, state, and local regulations, as well as facility policies.
This role is essential in ensuring clinical excellence, resident safety, and positive outcomes.
Qualifications
- Graduate of an approved LPN program
- Current, active, and unencumbered PA LPN license
- Current CPR certification
- Experience in long-term care preferred
- Knowledge of rehabilitative/restorative nursing practices preferred
- Experience with PointClickCare (PCC) or EHR systems preferred
- Strong communication and teamwork skills
- Must meet all background, health, and compliance requirements
Clinical Care
- Administer medications and treatments per physician orders
- Maintain accurate documentation in MAR/TAR and PointClickCare
- Monitor and report changes in resident condition
- Perform clinical tasks including:
- Vital signs and accuchecks
- Specimen collection
- Wound care and treatments
- Participate in admissions, transfers, and discharges
- Communicate with physicians and families regarding resident status
- Collaborate with the interdisciplinary team (IDT)
- Assist with care planning and quality improvement initiatives
- Direct and supervise CNAs and nursing assistants
- Provide coaching, guidance, and support to staff
- Ensure adherence to policies, procedures, and standards of care
- Ensure proper handling and documentation of controlled substances
- Follow infection control, PPE, and safety protocols
- Complete incident/accident reports per policy
- Maintain a clean, safe, and compliant work environment
- Maintain strict HIPAA compliance and confidentiality
- Treat all residents with dignity, respect, and compassion
- Respect resident choices and report concerns appropriately
- Ensure privacy and culturally competent care
- Ability to lift, push, and pull 25–50 lbs
- Ability to move throughout the facility during shift
- Ability to respond to emergencies and assist with evacuations
