JOB SUMMARY:

The Staff Pharmacist is responsible and accountable for the provision of broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy. Through various assignments within the department, this pharmacist provides support of inpatient and outpatient services and delivers optimal medication therapy to patients with a broad range of disease states. The Staff Pharmacist works in collaboration with multidisciplinary care providers to monitor, assess and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.

This position does not have direct reports.

ESSENTIAL DUTIES:

  • Performs all job duties and functions within the scope of practice for a Registered Pharmacist, as defined by the Oregon State Board of Pharmacy.
  • Practices in the assigned service and care area, and/or rotates through different service area assignments.
  • Provides oversight and ongoing supervision for all activities performed by supportive personnel (pharmacy technicians, clerks, and interns) to ensure compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.
  • Performs pharmacist tasks including compounding, drug therapy reviews, verification, patient counseling and education, and medication therapy management. Reviews, interprets, and accurately dispenses prescribed medications.
  • Applies evidence-based medicine, national guidelines, and hospital’s policies and procedures when creating recommendations to health care providers for optimizing the patients’ medication regimen.
  • Analyzes and reviews medication lists for potential patient safety problems, including drug interactions, suboptimal medication regimens, etc.
  • Researches, develops and implements drug utilization and disease state management strategies and intervention techniques to deliver high quality, cost effective healthcare.
  • Complies with policies and procedures related to preparation and supervision of all sterile and non-sterile compounding.
  • Accountable for safe medication storage, diversion monitoring and other key pharmacy inventory activities.
  • Documents all clinical activities and interventions accurately and completely.
  • Participates in education and training of healthcare practitioners (including students, interns, nurses, technicians, and other healthcare professionals).
  • Participates in quality initiatives to improve services by monitoring processes, documenting, and analyzing data.
  • Serves as a drug information resource to Patients, Providers and other members of heath care team.
  • Demonstrates knowledge of all current federal, state and local laws and regulations pertaining to the practice of pharmacy.
  • Demonstrates knowledge of and ability to assess patient needs, incorporating age and disease specific characteristics to optimize pharmacotherapy.
  • Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient and accurate.
  • Demonstrates medical literature evaluation skills and understanding of evidence-based medicine.
  • Maintains and enhances current knowledge and skills related to pharmacy and healthcare by reading pharmacy related journals, participating in educational opportunities, maintaining personal networks, and participating in professional organizations. Maintains awareness of developments in pharmacy care and pursues best practices that would enhance performance and health outcomes. Stays up-to-date on state/federal law and policy changes.
  • Demonstrates proficiency in hospital computer systems and software programs and applicable office equipment.
  • Displays ability to interact professionally and communicate clearly and effectively (verbal and written) with members of the pharmacy, other health care professionals and public.
  • Exhibits the ability to maintain confidentiality, think and act independently with minimal supervision.
  • Ability to organize and prioritize workload effectively, handle multiple tasks, and make critical decisions to ensure pharmacy services are provided in a timely manner.
  • Maintains regular, consistent and punctual attendance at the assigned job location.
  • Participates in department and staff meetings and committees.
  • Performs other duties as required or assigned.

QUALIFICATION REQUIREMENTS:

Education and License:

  • Graduation from an accredited College of Pharmacy with Bachelor of Science Pharmacy degree or Doctor of Pharmacy (Pharm.D.) degree required
  • American Heart Association Basic Life Support (BLS) Certification required (must be obtained within 60 days of hire)
  • Current Oregon Pharmacist license required

Certifications:

  • Relevant certification through Board of Pharmacy Specialties (BPS) preferred

Experience:

  • Minimum of 1 year of pharmacy practice experience preferred
  • Completion of ASHP accredited residency preferred

WORK ENVIRONMENT:

Works indoors in an air-conditioned office environment.

PHYSICAL DEMANDS:

Frequent use of computer and phone, and standard office machines. Must be able to stand, sit, stoop, twist, kneel, reach, push, pull using proper body mechanics and move/lift varying amounts of weight to a maximum of 50 lbs.

This is a full time position and is eligible for benefits.
