Summary:
The HIV Case Manager provides comprehensive case management services, including social service support and medical care coordination, to individuals living with HIV. This role ensures clients have access to high-quality, culturally competent care while promoting self-sufficiency, harm reduction, and overall well-being.
Responsibilities:
Frequent contact with patients, families, visitors, nurses and other department staff. Occasional requests to change work areas to cover another priority area to meet patient needs. Occasional stress and pressure caused by multiple simultaneous demands. May be exposed to various body fluids and/or infectious diseases. Universal precautions and techniques are included in orientation, and the availability of protective wear is supported and encouraged when the employee is at risk for exposure.

Summary:
The HIV Case Manager provides comprehensive case management services, including social service support and medical care coordination, to individuals living with HIV. Responsibilities:
- Maintain an active caseload of no fewer than 40 HIV-positive clients
- Conduct initial assessments, re-assessments, and develop individualized service plans
- Provide regular communication and follow-up with assigned clients
- Facilitate and support referrals for medical care, behavioral health, housing, food resources, and other social services
- Meet with clients in their homes and accompany them to medical and/or social service appointments as needed
- Coordinate care in collaboration with LCHC’s interdisciplinary HIV team
- Document all client interactions, visits, and communications in the electronic medical record (Epic)
- Assess and refer clients to Partner Services, Peer Support, and community-based resources as appropriate
- Deliver care within a Harm Reduction framework
- Conduct Positive Prevention discussions during clinical encounters
- Provide social and emotional support to individuals living with HIV/AIDS
- Promote client self-advocacy, independence, and engagement in health promotion activities
- Co-facilitate peer support groups and related activities as required
- Participate in scheduled administrative and clinical supervision
- High School Diploma or equivalent required
- One to two years of experience in community outreach and/or social services preferred
- CPR/BLS Required
- Knowledge of HIV care, community resources, and case management practices
- Ability to work collaboratively within an interdisciplinary team
- Strong documentation and organizational skills
- Ability to provide culturally competent care
- Bilingual/bicultural candidates strongly preferred (Khmer, Spanish, Portuguese)
