Job Description
The Health Promotion Nurse (HPN), in collaboration with the interdisciplinary asthma program team, will enroll and conduct asthma educational interventions, in the home setting, to older adult patients (65 ). The interdisciplinary team will consist of the HPN, bilingual/bicultural Community Health Workers (CHWs), Pharmacists, Primary Care Providers (PCPs) and UML researchers. The HPN will work with the team to develop effective strategies to improve asthma control, decrease urgent care utilization, address environmental and social barriers to improve asthma control, and reduce asthma triggers.
The HPN will utilize home visits, individualized home environmental action plans and educational interventions to improve the health of our older population with asthma. The HPN will also participate in Health Center community health educational events and screenings.
This is a two year grant funded position. The HPN will receive regular clinical and administrative supervision from the Director of Primary Care Services and Director of Health Promotions. The HPN will act independently as a registered nurse within the context of Massachusetts Nurse Practice Act.The HPN will have direct supervisory responsibility for CHWs within their scope of responsibility.
Qualifications
Current Massachusetts Registered Nurse, Bachelor’s degree preferred, Associates in Nursing required. Current CPR Certificate required. General computer knowledge: Microsoft Outlook, Office, Excel. Strong communication, verbal, written, and listening. Critical thinking, analysis, problem solving. Negotiating and conflict resolution. Motivated, self-directed and accountable. Function well is rapidly changing environment. Electronic Medical Record experience preferred. Bilingual preferred.
