Duties

• Conduct comprehensive intake assessments for veterans seeking assistance, utilizing motivational interviewing techniques to identify their needs and goals
• Facilitate access to social services, including addiction counseling, behavioral health support, disability resources, and workforce development programs
• Maintain accurate and detailed data entry records within case management systems while ensuring confidentiality and compliance with liability insurance requirements
• Provide exceptional customer service by guiding clients through the intake process, answering questions, and offering empathetic support during high-stress situations
• Collaborate with community partners and service providers to coordinate care plans tailored to individual needs, especially for underserved populations
• Manage crisis situations effectively by applying crisis management skills, de-escalating conflicts, and connecting clients with immediate support when necessary
• Document all client interactions thoroughly using strong writing skills to ensure clear communication across teams and compliance with social work standards

Requirements

• Proven experience working with individuals from underserved communities, including high-risk populations such as veterans with disabilities or behavioral health challenges
• Strong knowledge of social services, behavioral health practices, addiction counseling, and crisis management strategies
• Proficiency in Microsoft Office applications (Word, Excel, Outlook) for documentation and communication purposes
• Demonstrated ability to work effectively with culturally diverse communities while maintaining sensitivity and respect for individual backgrounds
• Excellent client communication skills coupled with active listening abilities to foster trust and engagement
• Experience in case management or social work roles that involve client assessment, service coordination, and follow-up care
• Familiarity with data entry protocols, confidentiality standards, liability insurance policies, and social service regulations
• Ability to handle sensitive situations with professionalism while supporting clients through high-stakes scenarios
