Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ conditions, noting changes in health status or behavior for medical records and communication with healthcare providers
- Support medication administration following proper protocols and documentation procedures
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record patient information and care activities
- Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer's disease
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
- Use medical equipment like Hoyer lifts, catheters, and vital signs monitors safely and effectively during patient care
- Follow HIPAA guidelines to ensure confidentiality of all patient information and maintain compliance with privacy standards
- Collaborate with nursing staff or other healthcare professionals in hospice care, long-term care settings, or home health environments
Requirements
- Proven experience working as a Personal Care Assistant or in similar caregiving roles within assisted living facilities, nursing homes, or home health settings
- Knowledge of medical terminology, medication administration procedures, and patient care techniques
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or dementia training are a plus
- Experience working with individuals with developmental disabilities or memory impairments preferred
- Ability to accurately monitor vital signs and recognize signs of medical emergencies
- Familiarity with medical documentation systems like EHR or EMR platforms for record keeping
- Strong communication skills to effectively interact with clients, families, and healthcare teams
- Physical ability to lift patients using Hoyer lifts or assist with mobility needs; comfortable performing cleaning and household chores as part of caregiving duties
- Valid driver’s license may be required for home visits; background checks will be conducted
