Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature to ensure health stability
- Support medication administration following proper protocols and HIPAA regulations to maintain confidentiality and safety
- Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are respected
- Use specialized equipment such as Hoyer lifts or resident’s lifts to safely transfer individuals with mobility challenges
- Observe and document patient conditions, behaviors, and responses to care plans to inform ongoing treatment strategies
- Maintain a clean and safe environment through regular cleaning tasks and adherence to infection control procedures
- Support residents in social activities or community outings to promote engagement and mental well-being
- Collaborate with nursing staff, social workers, and other healthcare professionals to develop and implement personalized care plans
- Ensure compliance with all regulations including HIPAA standards while maintaining patient privacy at all times
Requirements
- Proven experience working in assisted living facilities, nursing homes, group homes or home & community care settings
- Knowledge of Alzheimer’s care, dementia care, memory care, autism support or hospice care is highly desirable
- Valid certification in CPR and First Aid; additional training in medication administration preferred
- Ability to lift heavy objects safely and perform physical tasks required for resident transfers using equipment like Hoyer lifts or resident’s lifts
- Strong observational skills for patient monitoring and recognizing changes in condition or behavior
- Excellent communication skills to effectively interact with clients, families, and healthcare team members
- Familiarity with care plans development and documentation procedures in compliance with healthcare regulations
- Compassionate attitude with a genuine desire to improve the lives of individuals with disabilities or chronic health conditions
- Ability to work flexible hours including weekends or evenings as needed
