Responsibilities
As a Community Health Worker you’ll coordinate and provide services and resources to individuals/families necessary to develop and facilitate an intervention plan to ensure patient individual healthcare needs, goals and objectives are attained through telephonic and/or on-site screening process.
- Assess patient progress of intervention plan through interaction, monitoring, identifying variances, educating and problem-solving with the patient/family
- Facilitate appropriate utilization of resources through identification of alternatives, prevention of duplication/inappropriate use of services through on-going communication with patient/family and payer source
- Serve as a liaison between patients, providers, payers and community resources
- Other duties as assigned or requested
Qualifications
Job Requirements:
- High School Diploma
- One (1) or more years of recent related experience
Preferred Job Requirements:
- Associates degree or degree from an accredited college
- Bilingual – English/Spanish
- Experience in community, public or population health
- Experience in women’s health, obstetric care, infants/children, or supporting individuals with substance use needs
