Licensed Practical Nurse (LPN) – Home Health

Job Summary

The Home Health LPN provides skilled nursing services to patients in their homes under the direction and supervision of an RN and in accordance with physician orders, the plan of care, and agency policies. The LPN observes and reports changes in patients’ conditions, provides treatments and education, and documents care in the agency’s electronic medical record.

Essential Duties and Responsibilities

• Provide nursing visits and services according to the patient’s plan of care and physician orders.
• Monitor patients’ conditions, identify changes or concerns, and promptly report them to the supervising RN and physician as appropriate.
• Perform treatments, obtain ordered specimens, and administer medications as permitted by Kansas law, agency policy, and demonstrated competency.
• Teach patients and caregivers about medications, treatments, safety, and disease management.
• Document each visit and complete required clinical records within agency timeframes.
• Communicate and coordinate with patients, caregivers, the supervising RN, physicians, and other members of the healthcare team.
• Participate in care conferences and share updates about patients’ progress and needs.
• Participate in staff education and community outreach as assigned.
• Participate in a rotating on-call schedule, including nights, weekends, and holidays, as assigned.
• Perform other related duties as assigned.

• Comfort using computers, smartphones, electronic medical records, and navigation tools.

Education and Experience

• Active Kansas Licensed Practical Nurse license in good standing.
• Graduate of an accredited practical nursing program.
• Knowledge of medical terminology.
• Successful completion of required background and reference checks.
• Valid driver’s license and reliable transportation.
• Proof of current automobile insurance meeting Kansas requirements.
• Eligibility for employment in the United States; employment eligibility is verified through E-Verify.

Schedule and Workload

• Agency office hours are 8:30 a.m.–4:30 p.m. Monday through Thursday and 8:30 a.m.–1:00 p.m. Friday. Patient visit schedules may vary based on care needs.
• Evening or weekend work may be needed based on patient needs and assigned rotating on-call coverage.
• Salaried LPN positions have a weekly productivity expectation. Salaried employees do not use a traditional clock-in/clock-out schedule.

Travel

This in-person position requires travel to patients’ homes within a 30-mile radius of the agency’s Wichita office.

The LPN must be able to stand, walk, sit, use hands to handle or feel objects and equipment, reach, climb stairs, and communicate effectively. The position requires the ability to lift at least 30 pounds.
