Responsibilities:
- Conduct assessments to determine clients' social, emotional, and psychological needs.
- Develop and implement personalized care plans in collaboration with clients and their families.
- Provide counseling and support to clients and their families to navigate challenges.
- Coordinate with healthcare providers to ensure holistic care delivery.
- Facilitate support groups and educational workshops for clients and caregivers.
- Maintain accurate and confidential client records in compliance with regulations.
- Advocate for clients' rights and access to community resources.
- Stay updated on best practices and trends in social work and elder care.
Requirements:
- Master's degree in Social Work (MSW) or other related field
- or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting
- Strong communication and interpersonal skills with a compassionate demeanor.
- Ability to work collaboratively in a multidisciplinary team environment.
- Knowledge of community resources and services for seniors.
- Proficient in documentation and familiarity with electronic health records.
- Commitment to ongoing professional development and ethical practice.
Additional Requirements:
Current CPR and First Aid certification
Bilingual skills are strongly preferred (Vietnamese, Korean)
