Licensed Vocational Nurse - Private Duty Nurse

Duties

• Deliver comprehensive nursing care tailored to each patient’s needs, including medication administration, wound care, IV infusion, and catheterization.
• Perform detailed patient assessments, vital sign monitoring, and physical examinations to inform treatment plans.
• Manage advanced medical procedures such as ventilator support, tube feeding, and sterile techniques for infection control.
• Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
• Collaborate with physicians, case managers, and family members to develop and implement effective discharge planning and ongoing care strategies.
• Conduct triage assessments over the phone or in person to determine urgency and appropriate interventions for patients with acute or chronic conditions.
• Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those requiring memory care or hospice services.

Qualifications

• Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
• Proven experience in critical care settings such as ICU, emergency room, trauma centers (Level I & II), or medical-surgical units.
• Strong background in complex medical procedures including IV infusion, medication administration, phlebotomy, and catheter care.
• Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation.
• Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
• Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (long-term care), and individuals with disabilities or behavioral health needs.
• Ability to perform physical assessments and vital signs efficiently while demonstrating excellent patient care skills.
• Certifications in Basic Life Support (BLS) and sterile processing are preferred; additional certifications like PICU experience or hospice care are a plus.
