What are the responsibilities and job description for the Coordinator position at Lovable Family Home care?
A position in which you are expected to coordinate all patient services, follow up and ensure that all patient’s needs are met. Must be willing to speak to patients on a daily base.
Duties Include:
· Receive/coordinate and admit new members.
· Follow up on services of patients.
· Take aides vacation time
· Cover full member schedule.
· Inform patient and aide of any change in service. (Cover, Increase, Decreased, New aide and No Service)
· Following up on member’s that are on hold.
· Informing Auth department of expired, missing or reducing authorizations and Pt that are D/C.
· Calling case managers when authorizations are expiring.
· Inform supervisor of patients not receiving services and Why?
· Conferences with 15 days of occurrence.
· Enforce EVV within your patient list. Expect all aides to use the system.
· Be available on nights and weekends to answer any question about you patient.
· Inform scheduler of any changes in patients services, For Example: Holds, hospitals and deaths.
· Termination Forms
· Filling Out Incident reports correctly
· Sending sample notes to aides.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
