What are the responsibilities and job description for the FHC Patient Access Referral Specialist I position at Louisville Metro Government?
CHARACTERISTICS OF THE WORK
Under close supervision, performs all referral-related work, as well as interacts with multiple Family Health Centers (FHC) personnel and outside healthcare systems.EXAMPLES OF THE WORK (These statements are intended to be illustrations of the work of this class.)
Processes and manages all requests for referrals.
Verifies diagnosis codes that support medical necessity for referrals.
Schedules appointments for procedures and/or medical consults.
Prepares Transition of Care document in the electronic health record (EHR) and submits all necessary documents to referral office via direct message, when applicable, or through the fax server.
Communicates details of the referral order and referral management module in EHR and Practice Management System (PMS).
Queries ordering provider as necessary to obtain all information required by the referral office.
Closes out referral orders in EHR and the referral in PMS when the referral expires or results of visit are received.
Identifies patients with Limited English Proficiency (LEP) and/or other communication barriers and uses the appropriate interpretative services.
Documents as appropriate on the medical record, forms, and/or computer the required documentation related to interpretative services.
Adheres to standard precautions.
Performs related work as required.MINIMUM REQUIREMENTS
High school graduation or its equivalent.
One year of experience as a medical office specialist in a federally qualified health center (FQHC) or health care setting.
An equivalent combination of training and experience may be substituted, as determined applicable by Civil Service.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Responds to patient and FHC needs via telephone, internet, fax, computer software, and in person.
Lifts and carries materials weighing up to 15 lbs.
Spends extended periods of time sitting, standing, and walking.
Spends extended periods of time typing and reading computer screens.
Identifies medical record documentation, requests, and/or releases.
SPECIAL REQUIREMENTS
Must be available to work extended hours as necessary.
