Key Responsibilities
- Provide direct, community-based services to clients in their natural environments (home, school, or other community settings).
- Collaborate with families, support systems, and other healthcare providers to ensure holistic care.
- Maintain accurate and timely documentation in compliance with state and agency standards.
Qualifications
- Education: * Required: Bachelor’s degree in a Human Services field (Social Work, Psychology, Counseling, etc.).
- Preferred: Master’s degree in a Human Services field.
- Language: Bilingual (English/Spanish) is highly preferred.
- Experience: Previous experience in mental health or community outreach is a plus.
- Skills: Strong communication, empathy, and the ability to work independently in the field.
