POSITION PURPOSE

The Population Health Outreach Specialist plays a key role in supporting Population Health initiatives by engaging members, coordinating care, and optimizing scheduling workflows. This role leverages data to prioritize outreach efforts, improve access to care, and ensure completion of preventive services, contributing to improved health outcomes and program performance.

How You Contribute to the Company’s Mission In This Role

  • Conduct outbound and inbound member outreach to schedule Population Health visits and support provider schedules.
  • Educate members on the Population Health Program, preventive care, and recommended screenings to promote engagement and compliance.
  • Analyze reports and data to prioritize outreach efforts, optimize workflows, and meet departmental goals.
  • Coordinate services and appointments, including obtaining provider orders and scheduling screenings when visits cannot be completed.
  • Maintain accurate and timely documentation across multiple systems, including EMR and other internal platforms.
  • Support operational workflows such as insurance verification, demographic updates, routing support, and visit preparation materials.

Required Qualifications

  • High school diploma or equivalent required
  • Minimum of 3 years of experience in healthcare, managed care, health insurance, or customer service
  • Valid Louisiana driver’s license
  • Strong communication skills, including the ability to educate and engage members on clinical topics
  • Demonstrated ability to analyze data and adjust workflows to meet performance targets
  • Strong organizational skills with the ability to multitask and manage competing priorities
  • Proficiency in computer systems, including EMR and Microsoft Office Suite
  • Ability to maintain confidentiality and adhere to HIPAA requirements

Preferred Qualifications

  • Experience in Population Health, care coordination, or patient outreach roles
  • Familiarity with medical terminology and clinical workflows
  • Experience working with EMR systems and multiple data platforms
  • Clinical office or healthcare operations experience
  • Experience supporting performance metrics in a production-driven environment
  • Strong problem-solving skills with the ability to identify and recommend process improvements

Perform other job-related duties as assigned, within your scope of responsibilities.

  • Job duties are performed in a professional office or hybrid environment with typical noise levels.
  • Work is primarily completed while sitting, with occasional standing and movement required.
  • The employee may be required to lift and/or move up to 30 pounds and must be able to operate standard office equipment.
  • The ability to communicate effectively, document accurately, and manage multiple systems simultaneously is required.
