Loudoun Rehabilitation and Nursing Center is a 100-bed skilled nursing facility located in beautiful downtown Leesburg, VA. We have an exciting opportunity for someone to join our team as a Hospital Liaison!
Position Summary
The Hospital Liaison drives referral growth and market share for the Skilled Nursing Facility by building strong relationships with hospitals, physicians, and managed care partners. This role serves as a key representative, promoting the center’s clinical outcomes, PDPM performance, and value-based care capabilities.
Key Responsibilities
Grow referrals and market share through strategic relationship management with hospitals, physicians, and payer partners
Build and maintain relationships with case managers, discharge planners, social workers, and providers
Act as liaison to ensure smooth transitions of care and reduce readmissions and length of stay
Promote clinical outcomes, quality metrics, and PDPM performance to referral sources
Support physician engagement and managed care partnerships
Execute marketing strategies to increase census and business growth
Analyze referral trends and adjust strategies accordingly
Participate in meetings, events, and maintain CRM documentation
Qualifications
Bachelor’s degree preferred
2–3 years in healthcare marketing, hospital liaison, or business development
Knowledge of post-acute care, discharge planning, PDPM, and value-based care
Experience with managed care and payer relationships
Strong communication, relationship-building, and organizational skills
Self-motivated and able to work independently
Proficient in Microsoft Office and CRM systems
Valid driver’s license; local travel required
