Job Overview

Loudoun Free Clinic is a multi specialty office in Leesburg, VA. We are a professional, rewarding, inclusive office and our goal is to to provide healthcare services for adult uninsured Loudoun County residents who, as a result of economic or other barriers, would otherwise go without.

We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our dynamic team. In this vital role, you will have the opportunity to make a profound impact on individuals living with complex and/or chronic conditions, many of whom face multiple barriers to accessing care and need support in achieving their healthcare goals. This position requires flexibility and varies day-to-day to assisting patients telephonically or in person in settings such as the Clinic, in the community or at facilities.

This position is responsible for, but not limited to, the following duties:

• Serve as a cultural advocate and community connection point for LFC patients regarding LFC services and community opportunities.
• Work with the LFC team members to support patients facing inequities related to social drivers of health, including transportation, food insecurity, housing, public benefits eligibility, education, and other related barriers.
• Assist in identifying creative, innovative solutions to patient barriers and needs, including locating appropriate resources utilizing the UniteUs software platform, the internet, email and telephone.
• Coordinate specialty appointments for LFC patients with community based partners in health and empower patients to navigate those appointments independently by coaching them in self-advocacy.
• Advocate for patients when interacting with community non profit service partners.
• Provide input and recommendations to LFC staff regarding barriers to care and strategies to address equity challenges.
• Apply professional knowledge to support good judgment, decision-making, and effective outreach.
• Compile reports on referral activities as requested.
• Attend professional development events and training as identified and approved by the supervisor.
• Identify and maintain up-to-date knowledge of referral resources.
• Assist with Clinic reception, including answering and routing of calls, return of messages, greeting visitors and assisting patients when needed.
• Perform other related duties as assigned.

Qualifications

• High School Diploma/GED and/or related work experience required.
• Recent work experience in a community-based organization or healthcare setting preferred.
• Knowledge of human services, community resources, and
• Reading, writing and speaking in Spanish and English required.
• CHW certification preferred

Knowledge, Skills & Abilities

• Excellent computer skills required including MS365
• Excellent customer service and communication skills required.
• Ability to work flexible hours, including some weekends; reliable transportation required.
• Must enjoy working with people and problem-solving to ensure high satisfaction with service delivery.
• Ability to work collaboratively within a team-oriented environment.
• Exhibits professionalism consistent with LFC standards and policies regarding performance, behavior, and appearance.

Please note that this description is a just summary and additional responsibilities may be assigned as needed. LFC cross trains team members so we are able to cover duties when team members are out. Our current hours of operations are 8a-5p Mon/Tues and 8a-8p Wed/Th, and are subject to change.

Pay: $22.00 - $28.00 per hour
