POSITION SUMMARY:
The LPN in is responsible for several aspects of supervision of the residents, and staff. An LPN on the is responsible for maintaining the residents’ safety and care, overseeing the staff for the shift on job performance, and completion of their assignment. This is a position which involves the supervision of the Medication Techs, and Home Health Aides, as well as the coordination of services provided to the residents with other disciplines and providers in the community.
This is a Full-time hourly position in our Enhanced Care unit from 7am-3pm. You will work a 5 day on/2day off rotating schedule in which ever other weekend is a must.
PRINCIPAL RESPONSIBILITIES:

PRINCIPAL RESPONSIBILITIES:
- Assist in coordination and management of residents heath needs with the primary physician, resident, other care providers, and family members
- Supervise the intake of new medical/medication orders as well as distribution, and discontinuing of residents medication
- Accurate and appropriate charting in resident files
- Implement and ensure continued compliance with the policy and procedures
- Oversee/direct duties of staff
- Be knowledgeable with care needs of all the residents under the enhanced license.
- Meet customer service standards in a friendly, helpful, courteous manner with resident satisfaction being a priority
- Be knowledgeable of State Regulations (DOH)
- Perform case management tasks as necessary
- Monitor care needs and change in condition, report to Program Manager
- Supervisory responsibility for PCA’s, CNA’s Home Health Aides
- Participating in orientation, training, retraining of staff/new hires
- Monitor staff to ensure assignments and duties are completed
- Identify needs of staff if any additional training is needed
- Assist as needed with scheduling
- Assist resident care staff in providing care, and illness recognition as needed
- Maintain First Aid/CPR Certification
- All other duties as assigned
- Licensed LPN in good standing with the New York State Licensing Authority
- Previous experience in care for seniors preferred
- Case management experience
- Possess a valid driver’s license
- Moderate to heavy (50 pounds) expenditure of physical effort as it relates to lifting, carrying, pushing and pulling
- Must be able to routinely lift, carry, push/pull a minimum of 25 pounds
- Extended periods of time are spent walking or standing (on feet)
- Must be able to cope with the physical/emotional stress of the position
- Correctable eyesight must be 20/40
- Correctable hearing must be clear and acceptable at 5 feet
- High-risk exposure to blood borne pathogens
Salary : $30
