Are you ready to make a difference in the lives of others?
As a Speech Therapist, you will provide skilled speech therapy services by assessing, plannning, and implementing individualized therapy programs to help improve speech, language, and swallowing abilities. You’ll work with residents and patients to restore their communication skills and assist with eating and swallowing difficulties, all while providing compassionate, personalized care.
Key Responsibilities:
- Conduct comprehensive speech, language, and swallowing assessments for residents in SNF and home health settings
- Develop and implement individualized therapy plans tailored to the specific needs of each patient
- Provide direct speech therapy treatment, including exercises for speech, language, and swallowing disorders
- Collaborate with interdisciplinary teams to ensure a holistic approach to care
- Educate residents, families, and caregivers on strategies for improving communication and swallowing abilities
- Document therapy sessions and progress notes according to facility and regulatory standards
- Monitor and track patient progress, adjusting treatment plans as necessary
- Ensure all therapy activities are compliant with state and federal regulations
Must Have:
- Valid state Speech Therapist license (required)
- Strong knowledge of speech therapy techniques for various conditions, including swallowing and communication disorders
- Experience in a Skilled Nursing Facility (SNF) or Home Health setting (preferred but not required)
- Excellent communication and interpersonal skills
- Ability to work independently and as part of an interdisciplinary team
- Compassionate, patient-focused approach to care
Nice to Have:
- Experience with home health therapy services
- Familiarity with PointClickCare or other electronic health record systems
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
