Los Altos Post Acute is a 152-bed skilled nursing and rehabilitation center dedicated to providing exceptional post-acute and long-term care services. We are seeking an experienced, compassionate, and motivated Registered Nurse (RN) Unit Manager to join our leadership team.
As the RN Unit Manager, you will oversee the daily clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, and staff development. This position plays a key leadership role in promoting positive clinical outcomes, maintaining survey readiness, and fostering a culture of teamwork and excellence.
Responsibilities
- Oversee the daily clinical operations of an assigned nursing unit.
- Supervise and support RNs, LVNs, and Certified Nursing Assistants (CNAs).
- Ensure physician orders are implemented accurately and timely.
- Complete resident assessments and participate in interdisciplinary care planning.
- Monitor resident conditions and respond appropriately to changes in clinical status.
- Promote excellence in wound care, infection prevention, fall prevention, and skin integrity.
- Collaborate with physicians, therapists, dietary, social services, and other interdisciplinary team members.
- Audit documentation to ensure compliance with CMS, CDPH, and facility standards.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Assist with admissions, discharges, and resident transitions.
- Mentor and coach nursing staff while fostering a positive team environment.
- Maintain survey readiness and support regulatory compliance.
Qualifications
- Current, unrestricted California Registered Nurse (RN) license.
- Minimum one (1) year of RN experience in a Skilled Nursing Facility or Post-Acute setting preferred.
- Previous Unit Manager, Charge Nurse, or nursing leadership experience preferred.
- Strong knowledge of skilled nursing regulations, MDS processes, infection prevention, and resident assessments.
- Experience with PointClickCare preferred.
- Excellent leadership, communication, organization, and critical thinking skills.
- Current BLS/CPR certification.
