What are the responsibilities and job description for the Adult Day Out Coordinator position at Los Alamos Retired & Senior Organization?
Summary:
Under the supervision of the Executive Director, the primary responsibility of the coordinator is to ensure that the clients’ needs are accommodated to the greatest extent possible. The coordinator serves as the primary communication link between caregivers and the program. The coordinator ensures that the program is meeting all federal, state, and local guidelines.
Duties and Responsibilities:
- Develop and maintain service plans for all participants.
- Take and record vital signs on all participants on a weekly basis.
- Be able to learn and use the Well Sky-Aging & Disability program, formerly Mediware and SAMS=Senior Aging Management System.
- Input data daily into systems
- Be able to interview and fill out a full assessment of all senior participants on a regular basis as required.
- Maintain a daily attendance log and SOAP notes for participants.
- Provide weekly updates to family members.
- Assume responsibility for the day-to-day operation of the Day Out
- Prepare and distribute a monthly schedule for Day Out activities.
- Create in-house and field trip programming based on client interests and needs.
- Arrange for substitute staff members as needed.
- Plan and carry out staff training.
- Meet with caregivers on a quarterly basis or more frequently, if needed
- Perform intake interviews with clients and caregivers.
- Market the Day Out program and services to the community
- Collaborate with other programs and services to enhance the quality of life for participants.
- Work with Medicaid and other insurers as warranted.
- Perform such duties as required by the Executive Director
Minimum Qualifications:
Have a baccalaureate degree from an accredited institution of education in the field of social work, psychology, counseling, geriatrics, or a related health care field, or be a Registered Nurse, with 3 years’ experience and training in working with the elderly or special populations, and at least one year of supervisory experience.
Pay: $25.20 - $29.20 per hour
