Key Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accuracy and timeliness of RAI process documentation
- Facilitate interdisciplinary care plan meetings
- Oversee Medicare PPS and Medicaid reimbursement processes
- Monitor clinical documentation to support reimbursement and regulatory compliance
- Collaborate with nursing, therapy, social services, and medical providers to ensure quality resident outcomes
- Maintain compliance with CMS guidelines and long-term care regulations
Qualifications
- Current Maryland Registered Nurse (RN) License in good standing
- RAC-CT Certification, preferred
- Minimum of two (2) years of MDS Coordinator, Clinical Reimbursement, or RN Care Coordinator experience
- Strong understanding of:
- Medicare PPS
- Medicaid CMI
- Care Area Assessments (CAA)
- RAI Process
- Skilled Nursing Facility documentation requirements
- Excellent communication, organizational, and analytical skills
- Experience in long-term care, skilled nursing, or post-acute rehabilitation preferred
