SUMMARY
We are seeking a dedicated Medical Records Clerk to join our Medical Records Department. Reporting to the Manager of Medical Records, this position is responsible for a variety of essential tasks related to patient information management. Key responsibilities include collecting and verifying patient data, issuing and maintaining medical files, analyzing patient records for accuracy and completeness, and processing patient requests efficiently. The role requires attention to detail, strong organizational skills, and a commitment to maintaining confidentiality and compliance with healthcare regulations. This position plays a critical role in supporting medical records operations and ensuring that patient records are accessible and up-to-date.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Scans and analyzes a minimum of 20 inpatient/OBY, PT/OT, RRH and OP surgery records for chart sequences and physician deficiencies daily with 95% accuracy.
- Highlights all completed charts on Meditech discharge list.
- Responsible for scanning all loose material for discharge patient charts.
- Observes established safety measures to minimize hazards.
- Retrieve, processes and address medical record requests via email, mail or fax.
- Retrieve charts from designated areas and process in a timely manner.
- Answer phones, email correspondence and medical records requests.
- Manages own time in a productive and reliable manner completing work assignments on time.
- Completes appropriate volume of work while maintaining quality standards.
- Organize work procedures to general acceptable levels of productivity.
- Completes own work as required and will assist others as needed.
- Cooperates with coworkers and management to maintain harmonious environments.
- Adjust work schedule to accommodate changing priorities.
- Operates assigned equipment and performs all procedures in a safe manner.
- Utilizes proper body mechanics in all job functions
- Follows established emergency, infection control and for universal precautions procedures as required.
- Maintains work area and equipment according to standards and policies.
- Attends mandatory in-services related to safety and/or infection control.
- Supports and participates in departmental performance improvement activities.
- Provides performance improvements which enhance effectiveness and efficiency of department.
- Provides courteous service to internal/external customers in all activities.
- Offers assistance to all internal/external customers as appropriate and/or refers customer to appropriate individual or department.
- Communicates in a professional manner emphasizing objectivity and nonjudgmental viewpoints.
- Assist in solving customer problems in a cooperative manner.
- Identifies the attending physician for each inpatient record.
- Identifies consultants for each inpatient record.
- Assigns discharge disposition on all inpatient charts.
- Verifies patient information and determines location of charts.
- Retrieves charts from Meditech
- Other duties may be assigned.
COMPLIANCE RESPONSIBILITIES
- Understands and adheres to Loretto Hospital’s compliance standards as they appear in the Compliance Policy, Code of Conduct and Conflict of Interest Policy.
- Keeps abreast of all pertinent federal, state and Hospital regulations, laws, and policies as they presently exist and as they change or are modified.
- Maintains patience confidence by keeping patient records information confidential.

PHYSICAL DEMANDS
While performing the duties of this job, the employee is frequently required to sit, stand and walk as well as occasionally stoop, kneel, crouch or crawl. Using proper body mechanics, the employee may be expected to lift, pull, push or move patients and/or equipment. Fine motor skills are required when using hands and fingers to handle, feel or reach. Normal or corrected vision, hearing and ability to speak are necessary. Specific vision abilities required include close vision, sharp eye focus and ability to distinguish colors. There will be periods of stressful intense concentration with more than average pressure used in a variety of problem-solving situations.
