Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, ensuring accurate documentation in electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, and IV infusions following strict protocols and safety standards, including sterile techniques and aseptic procedures.
- Assist with medical procedures such as catheterization, wound suturing, spinal taps, and specimen collection/processing.
- Monitor patients’ conditions closely, managing acute pain, airway management, ventilator support, and responding promptly to changes in patient status.
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for patients with complex needs like dementia, Alzheimer's disease, or infectious diseases.
- Provide patient education on medication administration, nutrition, discharge planning, and health coaching tailored to diverse populations including pediatrics, geriatrics, and individuals with disabilities.
- Ensure compliance with HIPAA regulations and maintain accurate medical records while documenting all patient interactions thoroughly.
Experience
- Valid licensure as a Licensed Practical Nurse (LPN) in the appropriate state or region.
- Proven experience working in various healthcare settings such as hospitals (including ICU or Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for documentation purposes.
- Critical care experience preferred, including ICU or emergency room settings; experience with Level II trauma centers is highly valued.
- Familiarity with specialized care areas such as hospice care, palliative care, pediatrics (including neonatal ventilator management), or behavioral health is advantageous.
- Knowledge of ICD coding (ICD-9/10), CPT coding for procedures like Botox treatments or cardiac catheterization enhances your ability to deliver comprehensive patient care.
- Strong skills in patient assessment, medication administration—including injections and tube feeding—and infection control practices are essential for success in this role.
