The Opportunity

Care Model: One-on-one care with no double-booking

Caseload: Approximately 30–35 patient visits per week once established, depending on demand, visit mix, and schedule design

Schedule: Full-time Monday–Friday, typically 7 AM–4 PM or 8 AM–5 PM by mutual preference; no routine evenings or weekends; protected admin time

Compensation: $80,000–$100,000 base salary plus performance-based bonus opportunity

Start: Anticipated Q1 2027, coordinated around notice, licensure, and onboarding

Why Longevity

We are intentionally building a practice where excellent care and strong patient relationships come before visit volume. Our model gives clinicians time to listen, assess thoroughly, explain their reasoning, treat meaningfully, and help patients build long-term independence.

You will join early enough to shape a growing practice, but with established clinical standards and operating systems already taking form. Owner, Dr. Sara Hegemier, PT, DPT, provides clinical mentorship and oversees the role.

What You Will Do

• Perform comprehensive evaluations and manage patients from evaluation through return to activity and beyond.
• Develop individualized plans of care tied to meaningful functional goals rather than protocols, visit quotas, or insurance standards.
• Use movement analysis, strength and conditioning, therapeutic exercise, therapeutic activity, neuromuscular re-education, manual therapy, patient education, and other evidence-informed interventions.
• Provide dry needling when qualified, clinically appropriate, and aligned with patient goals.
• Clearly explain findings, recommendations, expectations, and the value of appropriate care.
• Maintain accurate, timely documentation and take ownership of patient follow-through and outcomes.
• Participate in patient-fit conversations with prospective clients and help them decide whether Longevity is the right clinical fit.
• Build trusted relationships within Charlottesville’s fitness, wellness, and healthcare community.
• Contribute to team meetings, case review, quality improvement, and the continued development of Longevity’s systems and standards.

Who We Are Looking For

Clinical ability is essential, but it is not enough on its own. The right person is someone patients quickly feel comfortable with and confident in—a clinician who listens well, communicates clearly, exercises sound judgment, and wants to help build a small practice the right way.

We’re looking for someone with:

• Strong evaluation, clinical reasoning, and exercise prescription skills.
• Comfort treating active adults, athletes—young and old—and people who want to remain active across the lifespan.
• Confidence practicing independently and collaborating when necessary for improved patient outcomes.
• Exceptional interpersonal, communication, and relationship-building skills.
• Comfort discussing treatment recommendations without pressure or overselling care.
• Comfort changing the plan of care as treatment progresses when necessary and clearly communicating the reasoning.
• A dependable, organized, accountable, growth-minded approach and comfort working in a small-business environment.

Qualifications

• Graduate of a CAPTE-accredited physical therapy program.
• Active Virginia Physical Therapist license or eligibility to obtain one before employment.
• Three or more years of outpatient orthopedic or musculoskeletal experience preferred. Exceptional newer clinicians with strong clinical judgment, communication skills, and alignment with Longevity’s model will be considered.
• Excellent written and verbal communication skills.
• Current First Aid/BLS certification.
• Advanced training or experience in dry needling, strength and conditioning, running-related injuries, sports rehabilitation, movement assessment, or performance-based physical therapy is strongly preferred.

Compensation & Benefits

• $80,000–$100,000 base salary, commensurate with experience, clinical expertise, and anticipated responsibilities.

Over time, the clinician builds a sustainable caseload through excellent care, communication, follow-through, and community reputation without compromising clinical judgment or patient needs—and strengthens the team and systems around them.
