Longevity Health is seeking a dynamic and experienced leader to serve as our Director of ACO Operations & Client Success. This role is central to advancing our value-based care strategy and driving the success of our ACO REACH High Needs model.
You will be responsible for ACO performance, provider partnerships, operational execution, and client success, working closely with physician groups, nursing homes, and post-acute providers. This is a highly visible leadership role that blends strategy, relationship management, and data-driven performance improvement.

You will be responsible for ACO performance, provider partnerships, operational execution, and client success, working closely with physician groups, nursing homes, and post-acute providers. What You’ll Do
ACO Growth & Partner Engagement
- Drive growth and sustainability of physician and medical group partnerships
- Develop and maintain strong relationships with PCPs, specialists, and provider organizations
- Lead onboarding and implementation of new physician groups and post-acute partners
- Build partnerships with nursing homes and assisted living facilities to expand ACO participation
- Translate strategy into scalable workflows, processes, and accountability structures
- Oversee EMR referral management and care coordination processes
- Manage alignment submissions and attribution processes across the network
- Lead vendor relationships supporting ACO operations and clinical initiatives
- Track and analyze attribution trends, growth, churn, and performance
- Deliver data-driven insights and reports to internal leadership and external partners
- Conduct regular performance reviews with providers and implement improvement plans
- Establish and manage KPIs across growth, onboarding, engagement, and retention
- Partner closely with clinical leadership to align on performance improvement initiatives
- Collaborate with ISNP and IPA product leaders on market strategy and education
- Support provider education on value-based care and government programs
- Support transition planning for the CMS/CMMI LEAD ACO model
- Evaluate opportunities for Convener participation and long-term ACO expansion
- Contribute to long-range growth planning and market expansion strategies
- Lead, coach, and develop ACO Operations & Client Success team members
- Foster a culture of accountability, collaboration, and continuous improvement
Required
- Bachelor’s degree
- 5 years of healthcare experience, including 3 years in leadership
- Strong knowledge of:
- Value-based care models
- Government programs (Medicare, ACOs)
- Provider practice and nursing home operations
- Experience managing provider relationships and driving performance outcomes
- Strong analytical, reporting, and presentation skills
- Proven ability to lead cross-functional initiatives and manage change
- Experience with ACO models, especially ACO REACH
- Strategic decision-making and problem-solving
- Relationship building and communication
- Operational excellence and execution
- Data-driven performance management
- Adaptability in a fast-changing environment
