  • General Purpose of the Job
     The Clinical Liaison serves as a dedicated point of contact between Long Island Select Healthcare group home agencies or individuals supporting individuals with intellectual and developmental disabilities (IDD). This role ensures seamless access to care through real-time communication, proactive outreach, and comprehensive coordination of both clinical and non-clinical services.



  • The Clinical Liaison is responsible for facilitating patient access, supporting adherence to care, coordinating services, and improving engagement and retention in care. This position plays a critical role in bridging communication gaps, improving outcomes, and enhancing the overall patient and caregiver experience.


  • Essential Duties & Responsibilities
     
     Access & Communication


    • Serve as the primary clinical contact for group home staff, providing immediate email chat, and phone access with timely response to requests and inquiries.

    • Establish and maintain routine touchpoints with assigned group homes to proactively address needs.

    • Triage incoming clinical and administrative concerns and route appropriately when needed.




    Care Coordination & Navigation



    • Navigate the organization’s Electronic Health Record (EHR) and scheduling systems to:

    • Schedule appointments and follow-ups

    • Coordinate referrals and specialty care

    • Assist with prior authorizations (PAs)

    • Process medication refill requests

    • Communicate with providers and ensure ongoing patient support

    • Ensure continuity of care across providers and settings

    Clinical Support

    • Address basic clinical inquiries within scope (LPN/MA level) and escalate appropriately.

    • Support adherence to treatment plans, medications, and provider recommendations.

    Heath Home & Patient Engagement



    • Actively engage and retain Health Home enrollees in care.

    • Conduct outreach to:


  • Re-engage patients who are disengaged from care

  • Educate and enroll eligible individuals into Health Home services


  • Transitional Care Management



    • Coordinate transitions from inpatient or emergency settings to home or other care environment.

    • Schedule timely follow-up appointments post-discharge.

    • Ensure patients remain connected to care and avoid preventable readmissions.



    Advocacy & Support



    • Serve as a patient advocate, ensuring needs are addressed across clinical and community systems.

    • Provide individual and family support, including education and guidance.

    • Act as liaison between patients, families, group homes, and community agencies.



    Community & IDD-Focused Support



    • Support individuals living in group home and community settings with IDD.

    • Collaborate with interdisciplinary teams to address complex, medical, behavioral, and social needs.


  • Qualifications & Experience

  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Education and/or Experience
     Certification as a Licensed Practical Nurse (LPN) or Medical Assistant (MA) required. Strong knowledge of coordination, referrals, and healthcare systems. Excellent communication and organizational skills. Ability to multitask and manage high-volume communication efficiently. Experience navigating EHR systems – Experience with eClinicalWorks preferred. Preferred experience working with individuals with IDD and/or in group home settings. Bilingual preferred.

    Language Skills 


    Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.




    Mathematical Skills 


    Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.




    Reasoning Ability 


    Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.


     


    Computer Skills  


    To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Outlook, Excel, Word), as well as Practice Management and Electronic Medical Record software – eClinicalWorks a plus).


    Certificates, Licenses, Registrations 


    Clean valid NYS Driver's license.

    Interpersonal skills, organization & planning, problem solving, conflict resolution ability, assertiveness, strong teamwork & networking skills, ability to carry out job duties independently in the community. In addition, attendance at all mandated agency management training activities. Demonstrated ability to work effectively with culturally diverse populations.

The employee must occasionally lift and/or move up to 25 pounds.

  • Means and Ability to travel between sites, patient homes, and group homes required.
