Position: Patient Care Coordinator
As a Patient Care Coordinator, you’ll play a vital role in welcoming new patients, guiding them through their journey, and ensuring every interaction is met with empathy and excellence. From coordinating appointments to managing essential patient data, your role will make a real difference in patients’ lives by helping them feel seen, heard, and cared for.
Key Responsibilities
- Support and Elevate the Patient Experience: Serve as a consistent, reliable point of contact for existing patients—helping them navigate appointments, forms, treatment questions, and care coordination with empathy and professionalism. Set the tone for excellence by modeling strong service standards and communication.
- Customer Service Excellence: Engage with patients across phone, email, website, and chat, ensuring every interaction is met with empathy and exceptional service.
- Data Integrity: Accurately enter and maintain patient information, upholding confidentiality and respect for their privacy.
- Collaborate Across Teams: Partner with clinical and administrative staff to ensure seamless care coordination and uphold our clinic-wide commitment to excellence.
- Contribute to Process Improvement: Identify opportunities to streamline tasks or enhance workflows, and bring forward ideas to elevate patient care and team effectiveness.
What We’re Looking For
- Professional Experience: Minimum 2 years in customer service, healthcare administration, or a related leadership-oriented support role.
- Communication Skills: Exceptional written and verbal communication skills are essential to engage with patients and staff with empathy and clarity.
- Tech Savvy: Experience with word processing, spreadsheets, email, scheduling, and data entry software.
- Detail-Oriented: Strong attention to detail to ensure accurate data collection and patient records.
- Time Management: Ability to prioritize tasks efficiently to maintain a smooth workflow.
- Team Player: Demonstrates initiative, accountability, and collaboration—willing to lead independently and contribute to team success.
