Licensed Practical Nurse (LPN) – Green House Model | Make a Meaningful Difference in Elder Care
Location: Palmyra, Pa
Shifts: 3pm to 11pm and 11pm to 7am (FT/PT) as well as FLEX & PRN
💲*$4.00/hour shift differential for 3PM–11PM & 11PM–7AM shifts*

About the Role:
In this unique setting, the Green House LPN ensures high-quality nursing care for elders while mentoring and guiding Shahbazim (care team members) on medical matters. You’ll foster a warm, home-like environment where elders receive not just care—but connection, comfort, and respect.
What You’ll Do:
- Coordinate and deliver exceptional clinical care, medications, and treatments according to physician orders
- Monitor and evaluate elder conditions, initiating interventions to maintain optimal physical and emotional well-being
- Document all care and observations accurately in electronic health records (EHR)
- Respond to medical emergencies with a calm, knowledgeable presence, respecting advanced directives and protocols
- Update and participate in individualized care planning and interdisciplinary meetings
- Communicate clearly with elders, families, Shahbazim, physicians, and fellow team members
- Support restorative nursing initiatives and promote elder involvement in daily activities and community events
- Oversee the administration and inventory of controlled medications
- Ensure infection control policies are upheld and team members follow safety standards
- Participate in continued education, training, and orientation for new staff
What You Bring:
- Current PA license as an LPN or RN in good standing
- Graduation from an accredited nursing program (or pending licensure with a valid Temporary Practice Permit)
- CPR certification (or ability to obtain)
- Strong clinical judgment, documentation skills, and leadership qualities
- A commitment to compassionate, person-centered care
