What are the responsibilities and job description for the Coordinator-Rehab Program-LVN position at Loma Linda University Health?
Job Summary: The Coordinator-Rehab Program-Licensed Vocational Nurse (LVN) is responsible for the accurate and timely completion of all Team Conference notes and continued authorizations as initiated by the Intake team. Oversees the coordination, management, and program development of assigned patient programs. Coordinates and case manages the patient's hospital stay from admission to discharge ensuring appropriate length of stay. Maintains ongoing communication between team members, families and the insurance company to ensure ongoing authorization for services. Documents case management activities and utilization review as per requirements. Regularly updates discharge arrangements flow sheet with accurate information in a timely manner. Ensures regulatory standards are met for all regulatory agencies. Provides weekend and holiday coverage as specified by the Manager. Performs other duties as needed.
Education and Experience: Graduate of a school of vocational nursing. Minimum of two years of experience as a Licensed Vocational Nurse (LVN) required. Minimum one year of experience in a Rehabilitation facility or skilled nursing facility (SNF) preferred.
Knowledge and Skills: Ability to organize and analyze data. Knowledge of how CMG assignment is calculated for assigned patient diagnosis. Able to: read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.
Licensures and Certifications: Active California Vocational Nursing (LVN) license required. Basic Life Support (BLS) certification from the American Heart Association required. California intravenous (IV) certification for designated areas preferred.
