Job Description
Job Summary: The Case Manager-Emergency Department (ED) serves as a key member of the patient care delivery team in a fast-paced emergency department licensed under Loma Linda University Children's Hospital (LLUCH), a Magnet Recognized academic tertiary care hospital with Level 1 Trauma Designation. Applies clinical knowledge and specialized emergency medicine case management, utilization review and care coordination methodologies and practices to a culturally diverse and complex patient population in partnership with physicians, nurse practitioners, physician assistants and multidisciplinary healthcare team. Utilizes clinical expertise, discretion and independent judgment to perform the nursing process (assessment, care planning, intervention, evaluation) independently and in accordance with LLUCH's established Professional Practice Model (Duffy's Quality Caring Theory). Collaborates with the physicians and multidisciplinary healthcare team to plan of care, resource utilization and timely discharge from ED with appropriate referrals. Utilizes effective interpersonal skills to maximize excellence in customer service, safe patient care and professionalism. Demonstrates ability to work well under stress in a fast-paced environment performing multiple tasks simultaneously. May be required to participate in on-call schedule based on business needs. Performs other duties as needed.
Education and Experience: Bachelor's Degree in Nursing required. Minimum three years of bedside nursing in hospital setting. Minimum one year case management experience preferred.
Knowledge and Skills: In-depth knowledge of case management practice and methodologies as applicable to emergency department and services. Exhibits professionalism in behavior, dress and conversation at all times. Able to speak, read and write legibly in English (and Spanish preferred) with professional quality; communicate effectively in person, in writing and on the telephone; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; relate and communicate positively, effectively and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach and collaborate; accept direction; think critically; use sound judgment based on factual information and clinical knowledge; work independently with minimal supervision; meet deadlines; perform basic math and statistical functions; manage multiple assignments effectively; organize and prioritize workload; problem solve in a proactive and creative manner; recall information with accuracy; pay close attention to detail; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records and written documents necessary to position; exhibit professional behavior at all times.
Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification issued by the American Heart Association required. Case Management certification preferred.
