What are the responsibilities and job description for the ACT Nurse position at Logan Mingo Area Mental?
This is a full time position as part of MLIH's ACT Team. The position involves primarily off site work in the community, including home visits. The position reports to MLIH's 3 Mile location in Logan, WV.
POSITION PURPOSE:
Must be knowledgeable about the evidence based ACT Model for adult behavioral health populations with willingness to promote and respect the philosophy of that model. Must be able to work collaboratively as part of transdisciplinary team in meeting the integrated physical and behavioral health and wellness needs of the persons served. Must have the ability to provide culturally competent care to diverse populations. Must demonstrate a commitment to the provision of strength based consumer-centered care and be supportive of the consumer’s own goals and motivation toward positive outcomes. Must be organized, able to manage competing priorities, utilize strong problem solving and conflict resolution skills, and possess the ability to work effectively independent and with a team setting.
ESSENTIAL JOB FUNCTIONS:
1. To complete physical health assessments for ACT Consumers.
2. Prepare medication boxes for weekly distribution and deliver medication boxes to consumers as needed. Call in prescriptions.
3. Complete lab work, urine drug screens, and documentation for Lab Corp.
4. Acquire prior authorizations for medications and injections and maintain medications and injections in appropriate locked closet.
5. Provides assistance and support to all team members in meeting the overall health and behavioral needs of consumers receiving psychiatric evaluation and medication management.
6. Participate with group therapies as needed and schedule appointments when necessary.
7. Work closely with insurance companies, funding sources, pharmacies, laboratories, and hospitals related to consumer medication concerns or discharge planning.
8. To educate, counsel, and support patients in relation to their behavioral health and physical health and wellness care. To reinforce policies and procedures that promotes the behavioral and physical health and wellness of patients.
9. Provide team training on health issues, including HIV/AIDS/TB and other infectious diseases to the organization’s established group programs.
10. Provides timely response to consumer calls regarding medication issues, problems, concerns, or questions.
11. Assist clinic operation with attaining patient’s Temperature, Pulse, Respiration, Weight, Body Mass Index (BMI) and injections.
12. To work collaboratively with the ACT physician and other providers in meeting the health and behavioral health needs of the ACT consumer.
13. Provides crisis intervention services in office and in the community. Provides emergency on call phone support to ACT consumers and team members as needed.
14. Provides timely, accurate documentation.
15. Participates in daily organizational meetings.
16. Participates in the development of policies and procedures related to health, medications, and psychiatric care.
17. Be a part of crisis through telephone and VTC modalities.
18. Assists in the overall team case management, counseling, and support functions.
19. To provide in-home, community, hospital, and transitional care supports/interventions to patients involved in project as needed.
20. To maintain accurate and timely information in the electronic health record (EHR) and support the mission of meaningful use measures.
21. To serve as a consultant to team on medication, health, and safety concerns.
22. Assist with ordering medical supplies and complete daily SALS.
MINIMUM QUALIFICATIONS:
Graduation from an accredited school of nursing and registered with the State Board of Examiners for Registered Nurses. One year as a professional nurse.
Must have a valid West Virginia driver’s license with a good driving record. Ability to function as a team member with good organizational skills. This position requires no history of ethical violations substantiated by a West Virginia licensing board.
