Our Transitional Care / Swing Bed unit is where recovery takes shape. Patients come here after hospitalization to regain strength, mobility, and confidence before returning home. It’s a 14-bed unit within our Critical Access Hospital, closely connected to our Medical/Surgical and Emergency teams. The work is steady, meaningful, and rooted in true patient connection. About the Unit You’ll care for patients with conditions such as stroke, dementia, traumatic brain injury, severe osteoarthritis, heart failure, COPD, and other chronic or degenerative illnesses. Each day combines clinical skill with personal care, helping patients take their next step toward independence. What You’ll Do Provide skilled, hands-on nursing care and coordinate individualized recovery plans Administer medications, perform wound and catheter care, and complete ongoing assessments Collaborate with therapy, providers, and families to support each patient’s goals Lead and support CNAs and other care partners within the unit Communicate clearly and compassionately with patients and their families The Details Beds: 14 total (shared with Med/Surg and ER) RN Ratio: 1:14, supported by 2 CNAs per shift Shifts: 8a–8p or 8p–8a Weekends/Holidays: Every other On-Call: None What We’re Looking For Graduate of an accredited RN program Current Montana or multistate compact RN license BLS required; ACLS, PALS, or NRP preferred At least 2 years of nursing experience preferred, ideally with long-term or rehab patients Why Join Us You’ll see the results of your work every day. Shift: Variable - 12 Hours (United States of America) Schedule: Logan Health operates 24 hours per day, seven days per week.
