Lock Haven Rehabilitation & Senior Center is seeking a dedicated and compassionate PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team. This position offers the opportunity to work with a collaborative interdisciplinary team in a Skilled Nursing and Long-Term Care setting focused on helping residents achieve their highest level of communication, cognition, and swallowing function.

Qualifications
Master's Degree in Speech-Language Pathology
Current Pennsylvania SLP license
Certificate of Clinical Competence (CCC-SLP) required
Strong communication and interpersonal skills
Commitment to patient-centered care
SNF/LTC experience preferred, but not required

Responsibilities
Evaluate residents with speech, language, cognitive, voice, and swallowing disorders
Develop and implement individualized treatment plans
Provide skilled therapy services to improve communication, cognition, and swallowing function
Educate residents, families, and caregivers regarding treatment goals and progress
Complete timely and accurate documentation
Collaborate with physicians, nursing staff, and rehabilitation team members
Participate in interdisciplinary care planning and discharge recommendations
