YOUR ROLE
You'll serve as a dedicated guide for families navigating dementia care, providing personalized, in-home support backed by Thalpos's coordinated care systems.
- Provide in-home care coordination and concierge support for dementia-affected families
- Maintain compassionate communication with family caregivers
- Document care activities within our platform
WHO WE'RE LOOKING FOR
- Experience in dementia care, home health, or elder care coordination
- Compassionate, professional, and highly reliable
- Comfortable working independently in a client's home
- Strong communication skills with families and care teams
- CNA, HHA, PCA, EMT, Social Work, or related certification a plus
