This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.
Job Description
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
- Partner with our neurologist(s) and broader care team to ensure continuity of care
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
- Active, unrestricted NP license in Virginia; certification in geriatrics, adult-gerontology, or family practice preferred
- 2 years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
