POSITION SUMMARY:
The Primary Unit Clerk performs assigned secretarial/clerical duties in the Geri-psychiatric Program. Provides administrative assistance to the Multi-Disciplinary Team. Working as the support person, they will undertake a variety of clerical and receptionist tasks. Also requires driving the designated vehicle to and from assigned destinations to pick up and deliver patients and/or supplies and may be required to assist patients in and out of vehicles.
PRIMARY RESPONSIBILITIES:
1. Initiate and maintain the patient insurance pre-certifications as indicated. Completes Medicare pre-certs for all patients.
2. Creates and maintains an on-going patient Treatment Plan Review schedule, to include patient diagnoses, and ensures every team member has an updated version
4. Ensures that all patient information and documents received are promptly scanned into the
medical record as they are received each patient day, ensuring they are readily available for staff.
5. Maintains a professional appearance and behavior in keeping with the interdisciplinary
professional team of which he/she is a member.
6. Communicates clear, concise, effective, accurate, and timely information verbally, written or
electronically (fax/email).
7. Responds with respect, tact, and diligence when responding to co-workers, patients, family and
visitors.
8. Complies with patient confidentiality in accordance with HIPAA at all times.
9. Attends all mandatory in-services, development and training sessions to stay up to date on current
policies and procedures.
10. Ensures patient demographic information is correct and updated in written and electronic
records in an on-going basis.
11. Enters daily patient charges and processes billing information.
12. Ensures patient rights are maintained at all times.
13. Completes organizational/unit daily paperwork. (ie. patient census, attendance sheets)
Position: Unit Clerk/Transportation
14. Completes unit supply order requisitions to maintain adequate quantity of forms, materials,
supplies and equipment, are available on unit.
15. Photocopies, collects and organizes documents, reports and files as required.
16. Uses professional telephone techniques when answering telephone, transferring calls and taking
messages.
17. Functions as a support during emergency situations.
18. Provides resources and support for patient before, during and after care, treatment and services
are provided.
19. Creates patient transportation schedules as applicable.
20. Performs & maintains monthly facility safety check to ensure facility is safe and reports
deficiencies to supervisor.
21. Maintains a clean, safe and functional work and patient care environment
22. Maintains/Updates patient scheduling.
23. Maintains patient discharge after-care/follow-up forms, schedule and call list as per policy. Makes
follow-up calls as required per policy and reports findings to the Primary Therapist or Program
Director.
24. Maintains the unit Fire Detector logs as per policy.
25. Maintains the unit Refrigerator logs as per policy.
26. Ensures the patient snacks, drinks (tea/coffee) and table is prepared and set up every scheduled
group therapy day. (Prior to leaving and picking up patients)
27. Double check charges for previous days through Cerner - these are available every morning for the
previous day through Cerner discern analytics to help make sure all charges went through.
28. Completes other duties as assigned by Program Director. Includes but not limited to helping with
community education.
TRANSPORTATION RESPONSIBILITIES:
1. Drives van as requested.
2 . Assists patient on and off the van following fall precautions. Walk patients to door of home and makes
sure patient is home safely. Must maintain a current driver’s license.
3. Drives van as requested, greets all passengers in friendly, courteous and helpful manner.
4. Responds to passenger requests conforming to procedure guidelines, listens politely and
attentively to all requests’ answering when appropriate, or communicating the request to
supervisor for direction.
5. Opens doors and physically assists all passengers on and off the vehicle.
6. Stresses the need for and enforces the use of passenger seat belts for passenger safety.
7. Arrives at work on time and when scheduled and prepares to start patient pickup/delivery runs on
schedule as specified by supervisor.
8. Maintains good working rapport with co-workers, supervisor, passengers and visitors. Listens and
responds in a positive manner to requests and/or inquires.
9. Communicates with passengers in a positive manner, if a disagreement should occur, attempts to
resolve in a calm and courteous manner and refers to supervisor for resolution of the
matter.
10. Ensures vehicle is safe and reports deficiencies to supervisor.
11. Maintains the van’s on-going mileage log.
12. Monitor, maintain and schedule the van’s maintenance needs.
13. Keeps vehicle clean, including rear view mirrors, windshield and inside, as verified by supervisor
or observed and reported.

14. Utilize cellular phone on an emergency only basis.
15. Picks up and delivers assigned passengers on time or notifies supervisor of any delays as they
develop.
16. Requests supervisor approval of any unscheduled stops.
17. Drives safely and courteously and complies with driving and traffic regulations.
18. Notifies unit immediately if any passenger becomes injured or ill in the vehicle. Prepares written
Incident Report and Statement Form by the end of the shift (as needed).
19. Notifies supervisor, as soon as safely permitted of any vehicle accident that may occur. Follows
directions of Law Enforcement Officers on the scene. Prepares written Incident Report and
Statement Form at the end of the shift.
20. Maintains a professional appearance and behavior in keeping with the interdisciplinary
professional team of which he/she is a member.
PROFESSIONAL REQUIREMENTS:
Completes annual educational requirements.
Maintains regulatory requirements.
Reports to work on time and as scheduled; completes work within designated time frames.
Wears identification badge while on duty.
Completes in-services as required.
Attends all staff meetings.
Meets program staff development standards.
Observes client confidentiality.
Must represent office in a professional manner to the public. This position is first person anyone talks with or sees when calls or visits office. Professionalism is very important.
LANGUAGE SKILLS:
Strong verbal and written skills.
EDUCATIONAL/EXPERIENCE:
High School Diploma or GED.
Experience in Behavioral Health, (preferred)
Basic knowledge of Human Behavior
CPR Certification (provided by LHHS)
CPI Certification (provided by UltraGroup annually)
Must complete Suicide Prevention Training (provided by Webinar annually)
Experience with Medicare and Billing (preferred)
