As a Registered Nurse, you will:

  • Work one-on-one with patients in a home environment
  • Computerized documentation
  • Provide Hospice care to the patient as per physician’s orders and by instruction to LVN’s, Home Health Aides, and other qualified staff.
  • Perform Responsibility for Patient Admission, Plan of Care, Re-certification, and Discharge.
  • Exercise strong decision making and judgment skills.
  • Collaborate and coordinate the care with the interdisciplinary team.
  • Work with an interdisciplinary team in weekly IDT meetings.

Responsibilities:

  • Assess patient/family for physical, psychological, social, and spiritual needs and to establish the plan of care, implement interventions.
  • Effective communication with attending physician, Medical Director, IDG to coordinate care and use of resources for the patient/family.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Provide quality of care by scheduling patient visits in accordance with plan of care.
  • Participates in IDG Meetings, patient / family meetings in coordination with IDG and Medical Director
  • Educate patients and their families on disease management, end-of-life care, and available resources
  • Promotes the Agency 's philosophy and administrative policies.

Qualifications:

  • Must be licensed in the State of Texas.
  • Diploma, Associate, of Baccalaureate degree in nursing
  • One year Hospice or Home Health care experience preferred.
  • Must be CPR certified.
  • Must work well independently, enjoy problem solving and understand the importance of accurate documentation.
  • Must have critical thinking skills.
  • Must have exceptional interpersonal skills.
  • Valid Driver’s license and proof of automobile insurance.
