We are seeking an experienced Home Health Registered Nurse to join our growing skilled nursing team in a unique hybrid role that combines direct patient care, clinical case management, and program development. This position is ideal for an experienced home health nurse who enjoys both caring for patients in the field and helping build exceptional clinical services behind the scenes.

About the Role
This is far more than a traditional field nursing position. Some days you'll complete an initial assessment for a new skilled client. Other days you may be providing post-operative nursing care, completing a 60-day reassessment, coordinating with a physician's office to obtain new orders, or supporting another nurse in the field. You'll also help ensure that every skilled client receives exceptional, consistent care throughout their episode of service. As one of the first dedicated Home Health RNs on our clinical team, you'll play an important role in helping shape and grow our skilled nursing program while working closely with agency leadership.
Responsibilities
- Complete comprehensive initial nursing assessments for new skilled home health clients.
- Perform required 60-day reassessments and change-in-condition assessments.
- Provide skilled nursing visits for select clients, including post-operative recovery patients.
- Develop, implement, and update individualized Plans of Care.
- Obtain physician orders and maintain communication with physician offices regarding client care.
- Serve as the clinical point person for assigned skilled cases, ensuring timely physician orders, care plan updates, regulatory compliance, and an exceptional patient experience from admission through discharge.
- Coordinate care with caregivers, therapists, physicians, families, and other members of the healthcare team.
- Support field nurses with clinical guidance, problem-solving, and resource coordination.
- Ensure accurate, timely documentation and compliance with agency policies and applicable regulations.
- Assist with occasional office-based administrative responsibilities and clinical coordination.
- Participate in community outreach, networking, and relationship-building efforts that support the growth of our skilled nursing services.
- Help develop and strengthen Living Care Home Services' concierge home health program.
Required Qualifications
- Current Pennsylvania Registered Nurse (RN) license in good standing.
- Minimum of three (3) years of Home Health nursing experience required.
- Demonstrated experience performing comprehensive home health assessments, reassessments, and change-in-condition visits.
- Experience developing Plans of Care and obtaining physician orders.
- Strong understanding of home health documentation, regulatory requirements, and clinical best practices.
- Excellent assessment, critical thinking, communication, and organizational skills.
- Comfortable working independently in patients' homes while collaborating closely with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and willingness to travel throughout Bucks County and surrounding communities.
- Experience with electronic medical records required; WellSky experience is a plus.
