Social Worker – Job Description:
The role of the Social Worker is to support residents make the transition from a previous living environment to life in a community setting either short term or long term. Social Services must support the resident and those closest to them. The Social Worker assures the residents continuing needs are met and that the person is given the opportunity to participate in planning for continued care at Sycamore, transfer or discharge back to their home.
Social Worker - Essential Duties and Responsibilities:
- Planning for pre-admission and discharge
- Providing psycho-social assessment and completion of relevant parts of minimum data set (MDS)
- Utilizing the minimum data set to guide the care plan
- Participating as a member of the interdisciplinary team in a resident care planning
- Invite and encourage resident attends their care plan and their preferences are honored
- Counseling residents and those closest to them
- Contact and utilize community resources on the resident’s behalf and serving as a link between the resident these resource systems when necessary
- Advocate for and protect Resident Rights
- Ensure the social and emotional needs of each resident are met
- Stimulate social contact and interaction
- Promote the maximum level of independence of each resident
- Ensure all residents are informed on Advanced Directives upon Admission and annually thereafter
- Promote volunteerism for Sycamore
- Marketing and providing tours for potential residents and families – when needed
- Work collectively with SNF team and ALF team when a resident is transferring within our community
- Plan an in-service annually for all employees on Abuse and Neglect and Resident Rights
- Follow Resident Rights’ policies at all times
- Come to work as scheduled and consistently demonstrate dependability and punctuality
- Follow appropriate dress code; conduct yourself in a caring courteous and professional manner
- Attend and complete in-service training programs
- Aware of the safety of all residents in order to minimize the potential for fire and accidents. Including: the community adheres to the legal, safety, health, fire and sanitation codes by being familiar with role in carrying out the community’s fire, safety and disaster plans and by being familiar with current Safety Data Sheets.
- Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
- Contribute to Sycamore’s efforts to maintain and/or improve quality of care through participation in the following: QAPI Committee
Social Worker - Leadership Responsibilities:
- Participate on various Committees within the organization
- Present during emergency situations and/or severe weather, as needed
- Ensure the Social Services Policy and Procedure manual is current and up-to-date
- Mentor team members on Resident Rights and Advanced Directives
- Participate in employee selection, hiring, training and termination for department
Qualifications:
- Bachelor’s Degree in Social Work
- 2 Years’ Experience in LTC setting
- Valid and current LCSW or LSW
- Excellent listening and communication skills
- Must have a genuine interest and desire to work with the senior population
