Job Description:
Summary
This person’s primary responsibility is to form longitudinal relationships and support LiveWell’s residential clients, their families, and LiveWell’s neighborhood teams. For success the person will use their professional expertise in the domain of Social Work to meet people where they are, create customized life plans, and promote wellbeing and resilience. This person will serve on LiveWell’s CoLeadership Team of Social Work, Nursing, Occupational Therapy, and Engagement professionals to provide a transdisciplinary approach to delivering supports, services, and a dynamic residential living environment best suited to meet individualized needs. The Social Work CoLead will work closely with the Client Services, Clinical, and Therapy Teams on transitions of care, life plan fulfillment, and a consistent service delivery model using a longitudinal and collaborative approach to ensure people are seen, heard, and valued.
Of note: This person will primarily support people living with dementia in Assisted Living. They will also support people transitioning from Assisted Living to Skilled Nursing for short term stays and/or long-term care while at LiveWell.
Duties/Responsibilities:
Leadership and Practice of LiveWell’s Core Values
· Be an active participant and contributor on LiveWell’s CoLeadership Team
· Design, execute, and implement organizational improvements, advancing LiveWell’s strategic vision, mission, and values
· Create connections to LiveWell’s values for all who live, work, and visit here
· Take an active role in LiveWell’s QAPI initiatives and various committees based on interest and professional expertise
· Participate regularly in continuing education and personal leadership development
Growth and Development of Neighborhood Teams
· Provide coaching and support to create an empowering environment for people living with dementia throughout various life stages
· Assist in staff evaluations in collaboration with the CoLeadership Team
· Sustain clear communication pathways between all stakeholders (People living with dementia, CNAs, Nurses, Families, etc.)
Support and Enhance the Wellbeing of those who Live, Work, and Visit LiveWell
· Deliver ongoing education to staff including, but not limited to, approaches to care, examining dementia through the lens of disability, and creating meaningful interactions focused on using social-emotional tools to advance wellbeing through end-of-life
· Build the team’s ability with a social work perspective to advocate for resident choice, attend to their individual preferences, and facilitate successful interactions based on learning life stories and building rapport between people, their environment, their families, and their care team
· Lead and facilitate a family support group, building formal and informal pathways for families to connect and creating longitudinal relationships with each of them
· Utilize social work lens while partnering with the CoLeadership team to identify and support residents, families, and staff as people advance in their diagnosis
· Support transitions for people living with dementia and their care partners using a holistic approach to meet people where they are, this may include new move-ins or transitions on campus
· Be an active presence on the neighborhoods, in partnership with the CoLeadership team, to model the approaches to care, foster the milieu, and deliver in the moment coaching focused on wellbeing with the resident at the center
Delivery on Professional Scope of Practice
· Complete portions of the Life Plan (service plan) for Assisted Living residents, participate in wellbeing rounds, and conduct assessments/provide emotional and psychosocial support in collaboration with the CoLeadership team
· Use a psychosocial lens to have people functioning at their best, continually monitoring that people’s overall needs are met
· Partner with the Clinical and Therapy Teams on providing emotional support to resident’s and their families due to change in conditions, transitions in care, disease progression, etc.
· Performs other duties, as needed/assigned.
Required Skills/Abilities:
Exceptional personal/human relations skills with an eye for human development, wellbeing, resilience, and life coaching
· Exceptional communication and organization skills
Education and Experience:
Professional experience working with people who are experiencing cognitive change and those who support people experiencing cognitive change
· Professional experience in long term care, assisted living, or homecare preferred
· Degree in Social Work
Physical Requirements:
Must be able to navigate various departments of the organization’s physical premises.
· Must be able to lift up to 15 pounds at times.
· Must be able to work with people of different cognitive abilities and support their wellbeing.
· Must be able to sit for prolonged periods of time.
Job Type: Full-time
Pay: From $70,000.00 per year
