We are seeking a skilled Physical Therapist to join our team at Hope Home Care. As a PT, you will be responsible for evaluating and assessing patient needs, formulating treatment plans, and providing therapy services in a home care environment.
Job Description
Key Responsibilities
- Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
- Administer physical therapy treatments as ordered by a physician.
- Direct and oversee treatment given by physical therapy assistants.
- Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.
Requirements
- A minimum of 2 years of physical therapy experience is preferred, with a preference for 2 years in an acute or post-acute care setting.
- Active state licensure in physical therapy is required.
