What are the responsibilities and job description for the Hospice Chaplain ~ Part Time position at Live Well Hospice, LLC?
Summary of Responsibilities: The chaplain is responsible for the provision of spiritual counseling to support the patient and family, consistent with their belief system.
Principle duties and accountabilities include, but are not limited to:
1. Participates as a member of the interdisciplinary team.
2. Provide direct spiritual support and/or counsel to patients/families in keeping with their beliefs.
3. Work with staff, clergy and community groups to enhance sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
4. With the IDT, develops the Plan of Care.
5. Provides bereavement follow-up services as defined in the bereavement Plan of Care.
6. Maintains proper records of visits to patients/families.
7. Makes contact with clergy or appropriate representatives of patients/families as indicated.
8. Performs occasional liturgical assignments such as memorial services.
9. Other duties as defined.
Qualifications:
1. An ordained pastor.
2. Demonstrated ability to be sensitive, flexible and ecumenical.
3. Requires reliable transportation, current driver’s license and current auto insurance.
Job Type: Part-time
Pay: $25.00 - $28.00 per hour
