Registered Nurse Navigator - Pediatrics Allergy

Role Overview:

This position involves providing patient education, coordinating care, and acting as a liaison between patients and healthcare providers to ensure seamless continuity of care. The role includes facilitating patient entry into care programs, supporting pre- and post-hospital care, and guiding patients through multidisciplinary clinics. It requires clinical support to assist in diagnosis and treatment, as well as collaboration with other providers to develop and implement personalized care plans.

Key Responsibilities:

• Coordinate patient activities, including intake, follow-up, and ongoing surveillance.
• Educate patients and families, reinforce understanding of conditions, and coordinate support services.
• Act as a patient navigator, managing communications and facilitating multidisciplinary team involvement.
• Maintain systems to ensure timely follow-up and provide nursing support during procedures.
• Educate team members and serve as a resource in clinical areas.
  
  

Qualifications & Skills:

• Graduate from a Specialty Training Program in Nursing; Bachelor's Degree preferred.
• Minimum of two years RN experience.
• Strong communication, coordination, and patient education skills.
• Ability to collaborate effectively within interdisciplinary teams.
