Primary Care Physician

Seeking a Primary Care Physician for an innovative PACE program serving individuals aged 55 with chronic illnesses or disabilities, promoting independence in their homes and communities.

• Provide comprehensive primary care, including assessments, chronic disease management, and preventive services.
• Collaborate with an interdisciplinary team to develop and integrate personalized care plans.
• Perform home and facility visits, ensuring coordinated, high-quality care.
• Contribute to team meetings, quality improvement, and clinical policy development.
• Maintain necessary licensure, DEA registration, and experience with elderly populations.
