Physician

Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.

• Join an interdisciplinary team to develop and monitor personalized care plans, providing geriatric and medical expertise.
• Supervise nurse practitioners and clinical staff, offering preventive, episodic, and chronic care, including physical and mental health assessments.
• Evaluate members, interpret diagnostics, manage medications, and coordinate referrals, ensuring tailored, comprehensive care.
• Educate patients and families on health maintenance and illness management.
• Travel locally as needed, with a schedule Monday to Friday, 8:00AM–4:30PM, with remote after-hours on-call rotation.
• Qualifications include a Doctorate in Medicine, licensure, DEA registration, Board Certification or eligibility in Family or Internal Medicine, and recent primary care experience with at-risk or elderly populations.
