Occupational Therapist
to deliver evidence-based rehabilitative care for adult patients recovering from serious illness or injury.
Key Responsibilities
- Assess patients’ functional abilities and rehabilitation needs.
- Develop and implement individualized treatment plans to improve independence in activities of daily living.
- Provide targeted OT interventions to support recovery and functional goals.
- Collaborate with an interprofessional team to coordinate care.
- Educate patients and families on strategies, goals, and home support needs.
- Document assessments, interventions, and progress in the EMR.
- Licensed Occupational Therapist with strong clinical judgment.
- Experience applying evidence-based rehabilitation principles.
- Ability to create individualized care plans and measure functional outcomes.
- Effective communication and patient/family education skills.
- Team-oriented mindset and ability to contribute to coordinated care.
- Proficiency with electronic medical records for accurate documentation.
