- Optimize pharmacotherapy to improve patient outcomes
- Perform medication order verification and conduct medication reconciliation
- Monitor medications for safety, efficacy, and therapeutic effectiveness
- Participate in antimicrobial stewardship efforts and formulary initiatives
- Educate patients and collaborate with care teams on medication use
- Support quality and safety programs and promote best practices
- Engage in continuous learning to maintain clinical expertise
